Student Payment Plan Enrollment

Complete this form to enroll in a Rescue Training Inc. tuition payment plan.

Before submitting this form, you must have:

• Submitted your program enrollment application
• Paid the required initial tuition payment
• Selected an approved payment-plan option

After we receive your form, Rescue Training Inc. will create your individual
Stripe payment schedule. You will then receive a separate secure email or link
from Stripe to enter your payment method.

Do not enter credit-card or bank information on this form.

Available Payment Plans

EMT Shift-Friendly Program
$650 initial payment, followed by two monthly payments of $625.

Advanced EMT Program
$600 initial payment, followed by three monthly payments of $550.

Paramedic Program
$1,200 initial payment, followed by eight monthly payments of $800.

Payment dates will be confirmed before the Stripe subscription is activated.
Students are responsible for maintaining a valid payment method throughout the
payment-plan period.
